What Changed on 23 December 2016
On 23 December 2016 the Ministry of External Affairs announced a landmark simplification of passport application requirements. The two headline changes affecting name updates were:

- 2.Self-declaration annexures introduced: A short set of prescribed annexures replaced the Gazette + newspaper requirement for common cases (marriage, divorce, minor corrections).

1. Register on Passport Seva
Go to passportindia.gov.in, register (or sign in), and start a Re-issue of Passport application. Choose "Change in Existing Personal Particulars" as the reason.
2. Book a PSK / POPSK slot
Book an appointment at your nearest Passport Seva Kendra or Post Office Passport Seva Kendra. Tatkaal slots are available if you need urgent processing.
3. Visit PSK with your Gazette
Carry: printed e-Gazette copy, original current passport, Aadhaar, PAN, application receipt, and passport-size photographs. The officer scans the Gazette as the name-change proof.
4. Police verification & delivery
Post-issue Police Verification for most cases. New passport is delivered by Speed Post; the old passport is cancelled (with 'Cancelled' punched on it) and returned to you.
